Virtue Mobile Care is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of Protected Health Information (PHI), provide you with this Notice of our legal duties and privacy practices, and notify you following a breach of unsecured PHI.
Understanding Your Health Record
Each time you visit a Virtue Mobile Care provider, a record of your visit is made. This information serves as:
- A basis for planning your care and treatment
- A means of communication among health professionals
- A legal document describing the care you received
- A source of information for billing and reimbursement
- A tool for quality assessment and improvement
How We May Use and Disclose Your Health Information
For Treatment
We may use and disclose your health information to provide, coordinate, or manage your healthcare. This includes coordination with third parties and referrals to specialists.
For Payment
We may use and disclose your health information to bill and collect payment for services, including verification of insurance coverage and claims submission.
For Healthcare Operations
We may use your information for quality assessment, training programs, licensing, accreditation, and business management activities.
Other Permitted Uses and Disclosures
