Our approach to value-based care begins with being present: understanding the patient, identifying care needs, and working with others involved in their care. We aim to support prevention and thoughtful follow-up, rather than relying only on a response when a problem has escalated.
We work with health plans, facilities, and healthcare organizations to discuss services that fit their patients and operational needs. Responsibilities, geographic availability, and service arrangements are confirmed for each partnership.
Facility collaboration.
Mobile visits can support patients in assisted living, skilled nursing, board and care, and other residential settings. We coordinate with the care professionals already involved and discuss how visits and follow-up will be arranged.
Partnership conversations can cover patient referrals, communication, clinical documentation, and the scope of services. We do not promise a particular change in utilization, cost, or patient outcomes.
Hospice collaboration.
We collaborate with hospice agencies seeking in-person patient visits and support with face-to-face documentation. Our providers can see patients at home or in facilities, with responsibilities and requirements clarified with the agency.
The goal is practical collaboration with the hospice team while respecting the patient’s comfort and care plan. Specific coverage, scheduling, and documentation arrangements are discussed before services begin.
